Insurance
Currently, I am accepting:
- Private pay clients
- Clients with United Healthcare insurance, also known as United Behavioral Health and Optum
- Clients with Group and Pension Administrators (GPA) insurance
- Clients with out-of-network benefits with Aetna
* I am no longer accepting Blue Cross Blue Shield insurance
Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify how your plan compensates you for psychotherapy services.
I’d recommend asking these questions to your insurance provider to help determine your benefits:
- Does my health insurance plan include mental health benefits?
- Do I have a deductible? If so, what is it and have I met it yet?
- Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
- Do I need a referral from my primary care physician in order for services to be covered?
I can verify your insurance benefits and I can bill them directly. You would be responsible for your copay and possibly a deductible (depending on your policy).
I no longer accept these insurance companies: Cigna, Value Options, Beacon Health, or MHN insurance.
Payment
I accept cash, check and all major credit and debit cards as forms of payment.
Cancellation Policy
If you have not notified us at least 24 hours in advance of a cancellation, you will be charged $25. There is a charge of $50 for a “Missed Appointment.” A “Missed Appointment” is a cancellation where there is no notification or where notification is made with less than two hours notice. Cancellations can be made on the online calendar (preferably) or by text to 239-281-4438, email, or voicemail.
